Allocation Panel Meeting - Thursday, April 12 4:00 p.m. @ Calhoun Depot (a representative from your agency is required to attend to meet with the panel.)
-Note for a Current United Way Agency receiving funding: There will be documents that you will need at the end of this form to complete the application before you submit. The documents requiring signatures must be signed, scanned to your computer and uploaded to this form. These have been emailed to the main contact person at each agency.
-Note for a Prospective United Way Agency applying to receive funds: Please email Vickie (firstname.lastname@example.org) before you start the application process because there are documents you will need at the end of this form to complete the application before you submit. The documents requiring signatures must be signed, scanned to your computer and uploaded to this form. Please note that United Way of Gordon County Allocation Committee will make the decision to include or not include your organization in the allocation process this year. Should the committee decide to fund the organization, you may be asked to participate in the allocation review process which involves a visit to the organizaiton by a panel of local volunteers who will help determine the funding amount. If chosen your organization needs to be available on April 12 at 4:00 p.m. at the Calhoun Dept to meet with the allocation panel.
-It is important to note that the information filled out on this form should only include numbers served from GORDON COUNTY
